Community Education Coordinator (1 full-time, non-exempt position)
Position reports to: Director of Community Relations
Salary: $17 per hourPosition Summary:
The Community Education Coordinator will increase patient access to SBFHC's services through outreach and educational activities to individuals, medical groups and community based organizations regarding the preventive health initiative offered by SBFHC, including those related to congestive heart failure, diabetes, and chronic disease management.Job Responsibilities:
- Assist in the assessment, planning, arranging, and implementing of psychosocial interventions.
- Perform short term counseling and crisis intervention, as necessary.
- Conduct a limited number of home visits to older patients, if needed.
- Spearhead education and counseling programs for both individual patients or groups of patients and serve as liaison to community, individuals and groups as appropriate.
- Counsel patients and interpret to professional and lay groups the special psychosocial needs of patients by identifying psychosocial disorders.
- Provide support to patients who want to improve health outcomes.
- Collect information related to psychosocial status and patterns of population groups as directed.
- Review and evaluate recent psychosocial health research and interpret it to professionals and patients.
- Prepare psychosocial educational materials as needed or required.
- Coordinate with clinical, health education, community health and other staff members in the provision of services to patients. Provide referrals for services needed.
- Coordinate with other organizational entities involved in psychosocial and mental health services.
- Participate in staff meetings.
- Performs other duties as assigned.
Qualifications:
- Bachelors Degree or equivalent experience.
- Prior experience in marketing or sales to diverse populations.
- Prior experience in marketing or sales to diverse populations.

Pediatrician (1 part-time - 0.60 FTE, exempt position)
Position reports to: Chief Operations Office/Chief Medical Officer
Salary: $75,600 pr/yrPosition Summary:
A Pediatrician will provide clinical case management support to staff providers and provide primary pediatric care to SBFHC's patients; interface with management to provide clinical input on the development and implementation of policies and procedures; meet with staff providers on an ongoing, regular basis to discuss concerns and opportunities for care improvements; perform patient assessments, physical examinations, order/perform necessary laboratory and diagnostic tests, proficiency testing, prescribe and dispense medications in compliance with SBFHC's established medical protocols.Qualifications:
- Valid license in state of California with no pending or previous disciplinary action from any state licensing entity.
- Current Board Certification in Pediatrics/li>

Patient Care Specialist (2 full-time, non-exempt position)
Position reports to:
Social Services Director, Clinic Services Manager
Salary: $18.50 pr/hrPosition Summary:
Under the direction of the Social Services Director, the Patient Care Specialist will coordinate with SBFHC's clinical staff to support patients to receive the comprehensive care and referrals essential to health and wellness. Responsibilities include case management of specialty and managed care medical referrals, and linkages to community based partners and educational services.Qualifications:
- Bachelor's Degree or equivalent required.
- Bilingual English/Spanish required.
- Detail oriented, strong organizational skills, and ability to multi-task.
- Basic computer skills, including windows based software programs.
- Excellent written and verbal communication skills.

Behavioral Health Specialist (1 part-time position - 0.40 FTE, exempt position)
Position reports to:
Psychologist, Social Services Director
Salary: $22,667 per yearPosition Summary:
The Behavioral Health Specialist is responsible for working in concert with SBFHC's Psychologist, Social Services Director, Clinicians and Patient Care Specialists to provide behavioral health and psychotherapy to the patients of SBFHC's primary care program, as well as to underserved community members in need of mental health services.Job Responsibilities:
- Establish partnerships with community organizations that serve SBFHC's target population;
- Develop and deliver educational presentations at community organizations;
- Support access of SBFHC's medical services by community members in need by providing payor program enrollment assistance and appointment scheduling at community partner locations.
- Requires local travel.
Qualifications:
- Masters Level Social Worker with valid LCSW license from the state of California with no pending or previous disciplinary action from any state licensing entity.
- Experience working with underserved populations preferred.
- Knowledge of current educational and psychosocial intervention procedures.
- Knowledge of basic technology and technological systems.
- Ability to perform clinical responsibilities within the organization's established guidelines in an organized, efficient manner.

Clinic Services Manager (1 Full-time, exempt position)
Position reports to:
Chief Operations Officer
Salary: $47,000 pr/yr
Position Summary:
Team Player with professional demeanor and strong leadership skills will assist the Chief Operations Officer (COO) in the operation of the clinic. Will oversee the performance of all patient care responsibilities including, but not limited to: appointment scheduling; patient check-in and check-out process; preparation of patient/client for clinician visit; medical record management and filing; performance of general nursing duties; tracking and filing of diagnostic studies; completion of end-of-day closing process. In conjunction with the COO, supervises the clinic staff, interviews and hires staff, and reviews staff performance. Facilitates the smooth flow of clients and patients to insure minimum patient waiting time and works within the organization by interacting well with patients, clients, staff members and providers.Qualifications:
- Minimum 3 years administrative and supervisory experience, including hands-on management of health care or medical practice - experience managing primary care practices preferred.
- BA/BS in related field or commensurate experience.
- Knowledge of modern outpatient clinic methods; basic medical assessment techniques; medical equipment and instruments used to administer care; skill and ability with regards to observing pertinent detail (i.e. reading thermometers, blood pressure devices, and other equipment gauges).
- Knowledge of common safety hazards and the precautions necessary to establish a safe work environment.
- Skill to supervise and manage employees and the clinical services to achieve the delivery of smooth flowing, quality patient care services; to perform assigned responsibilities with minimum supervision; to make appropriate patient care decisions; maintain quality control standards; to interpret, adapt and apply guidelines and procedures.

Salary: $12.50 pr/hrPosition Summary:
The Medical Assistant will prepare patient/client for clinician visit; review medical assessment and history forms; perform general nursing duties and oversee receipt of diagnostic studies; provide assigned program health education information within funding program guidelines to SBFHC clients and community members; facilitate the smooth flow of clients and patients and work within the organization by interacting well with patients, clients, staff members and providers.Qualifications:
- High School Diploma or GED required.
- Certificate of completion from an accredited Medical Assisting program.
- At least 6 months work experience in a clinic, medical office or other health care facility.

Mid-Level Clinician (1 full-time, non-exempt position)
Position reports to:Chief Operations Officer/Medical Director
Salary: $39.90 pr/hrPosition Summary:
The Mid-Level Clinician will provide medical care to patients seeking services at South Bay Family Health Care (SBFHC). They will perform assigned responsibilities under the supervision of the Medical Director and Chief Operations Officer in accordance with SBFHC's established medical protocols and in compliance with government agencies medical protocols and guidelines; participate as a member of the clinical team responsible for peer review, clinical policy and procedure implementation and maintenance; facilitate a smooth flow of patients; and work within the organization by interacting well with clients, patients, staff members and other providers.Qualifications:
- Current California license as a Nurse Practitioner or Physician Assistant.
- Current furnishing number or DEA certificate, as appropriate to licensure.
- One year clinical experience in area of specialty.
- Current CPR certification.
- Knowledge of state-of-the-art medical scientific and treatment methods in area of specialty, and an awareness of current medical, educational and psychosocial intervention procedures.
